Medical
Comprehensive, affordable medical coverage for you and your family — with plan details that depend on where you work.
Your medical plan options
You can enroll yourself and your eligible dependents in medical coverage if you are an active full-time employee working at least 30 hours per week.
You can choose from two Consumer Directed Health Plans (CDHP) and one Preferred Provider Organization (PPO) plan:
- CDHP $3,500
- CDHP $2,000
- PPO $850
Coming January 1, 2027
For 2027, the National and Indiana plans keep the same deductibles, out-of-pocket maximums and copays. Medical premiums go up about 3.5%, and the Dallas and Tyler, Texas plans are changing. See what’s changing for 2027.
Plan details depend on where you work
Coverage details and provider networks for each plan differ by location. Choose yours:
- National medical plans — all locations except Dallas and Tyler, Texas. Anthem network with Open Access hospital coverage.
- Indiana medical plans — if you work in Indiana, you can choose the National plans or the Local Indiana plans with Parkview Health, Tier 1 and Tier 2.
- Texas medical plans — Dallas and Tyler, Texas only. Imagine Health, Tier 1 and Tier 2.
How medical coverage works
The premium is the amount deducted from each paycheck to have medical coverage.
The deductible is the amount you pay before the plan starts to pay. The out-of-pocket maximum is the most you will pay for covered expenses each plan year.
Once you meet the deductible, the plan shares the cost of care with you. You pay either a percentage of the cost, called coinsurance, or a fixed fee, called a copay.
If you meet the out-of-pocket maximum, the plan pays 100% of eligible costs for the rest of the plan year. Deductibles and out-of-pocket maximums reset on January 1.
Plan highlights
All three plans include prescription drug coverage and cover 100% of in-network preventive care — annual checkups, screenings, exams and vaccinations.
- CDHP $3,500 and CDHP $2,000 have lower premiums and higher deductibles. You pay less each paycheck but more out of pocket before the plan begins to pay. Both can be paired with a Health Savings Account that Dexter contributes to. A good fit if you and your dependents are generally healthy.
- PPO $850 has higher premiums and lower deductibles. You pay more each paycheck but less out of pocket. A good fit if you expect regular health care use and prefer predictable copays.
- On both CDHP plans there are no copays — you pay the full cost of care until you meet the deductible, excluding preventive exams and maintenance prescriptions.
Cost for coverage
You pay premiums for medical and prescription drug coverage through paycheck deductions. What you pay depends on the plan you choose, who you cover, and whether you earned the wellness credit.
Medical premiums are the same regardless of your work location.
