Medical Plan Programs
Your medical plan comes with partners who help you find care, avoid overpaying and manage ongoing conditions — most at no cost to you.
Price protection through Imagine360
Your medical plan has built-in price protection so you do not overpay for care. Rather than using a specific provider network for hospital services, the plan pays providers based on a set price for each medical service. Quantum Health works with Imagine360 to make sure claims do not exceed the plan’s allowable limits.
If a provider sends you a bill that exceeds the out-of-pocket responsibility shown on your Explanation of Benefits, tell your Quantum Health Care Coordinator as soon as possible. They will connect you with an advocacy expert at Imagine360 who works on your behalf to resolve the bill.
What to tell a provider about your coverage
If you are enrolled in a National plan: The National plans do not use a provider network for hospital services, so some providers may not recognize your coverage right away. When you schedule or check in, tell them:
“I don’t have a provider network, but my plan pays providers directly based on fair, industry-standard rates. If you have any questions about billing or need help verifying my benefits, please call the Quantum Health Care Coordinators.”
You can also show your medical ID card — it has everything the provider needs to reach your Care Coordinators.
Preventive care at no cost
Annual medical and dental checkups are covered in full when you use in-network providers. Services include:
- Annual physicals — yearly checkups to assess overall health and catch issues early
- Routine screenings — blood pressure, cholesterol and other age-based tests
- Recommended immunizations — flu, tetanus, Covid and more, based on age and needs
- Well-child check-ups — growth, development and recommended childhood vaccines
- Preventive care — mammograms, prostate exams and other screenings
- Colorectal screenings — colon cancer screenings recommended beginning at age 45
