Provider networks
Open Access providers, sometimes called out-of-network providers, do not have set prices with the Dexter plan. To make sure you do not pay too much, the plan uses Price Protection — every claim is reviewed and payment is based on the fair price of the service.
You will carry two ID cards:
- One for professional and ancillary services plus prescriptions
- A second for in or outpatient hospital and facility services plus prescriptions
Both cards carry Quantum Health and pharmacy details.
Search the Anthem network for doctors, specialists and ancillary providers near you. Hospital and facility care is Open Access — you can use any hospital, and the plan protects you on price.

How the family deductible works
- CDHP $3,500 and PPO $850: each covered family member must meet the individual deductible until the overall family deductible has been met.
- CDHP $2,000: the overall family deductible must be met before the plan begins to pay.

If you or a covered dependent need bariatric, spine or joint surgery, you must receive care through the Carrum Health network. Call 888-855-7806 or visit carrum.me/dexter. Surgery of these types performed outside Carrum will not be covered.
